Discharge often comes faster than anyone expects, and the first two weeks home are when help matters most. Short-term care can carry someone from the hospital bed back to their own routine.
What recovery at home often needs
- Help getting in and out of bed, and to the bathroom, safely.
- Keeping to the discharge plan: medication times, exercises and follow-up visits.
- Meals, fluids and rest while strength comes back.
- Rides to follow-up appointments.
- Somebody there at night for the first few days.
Before discharge
- Ask the hospital team what help is needed and for how long.
- Ask whether any skilled care — wound care, injections, therapy — is being arranged through a home health agency.
- Plan the first night home before the day of discharge.
Care that ends
